New Patients

Are you a new patient?

New patients are required to be at the practice 15 minutes prior to the doctor’s appointment time.

We require you to complete a New Patient Information & Consent Form.

To ensure you are not delayed for your doctors appointment, we ask that you complete your new patient form online.

Parking

Practice parking is available on the right hand side of the road as you drive towards our building, on Graham Murray Place.
For those injured or unable to walk distances, closer designated parking has been reserved behind the building.
  • Please continue to drive to the boom gates located on the left hand side of the main building entry.
  • Press the touch screen and then NQPC
  • This will ring reception who will then open the boom gates
  • Drive through to allow you to drive through and park in one of the  labeled Sports Clinic NQ car parks

After Hours Entry

We wish to notify patients that due to the building not being managed by our clinic, there are times when the main entry doors will be closed and therefore patients and visitors will need to use the After Hours Entry Door to get to us.
  • Follow the path to left of the entry doors (past the boom gates)
  • Tap the security display screen (Welcome)
  • Select NQPC (This will call reception who will unlock the door)
  • Pull the door open and go up the Lift to Level 2

Important Information.

Please ensure you read through the following information below before proceeding to book an appointment online.

Our practice operates on an appointment system with 30 – 40 minute time slots, depending upon the doctor that you see.

Appointments can be made in person at the receptions desk, by telephone or online via your computer or mobile phone.

If you require longer appointments, please ask at time of booking. For example, if you have complex conditions, multiple issues or counselling.

On arrival, it is important to let reception staff know you are present by either presenting at the reception counter.

Due to the nature of our Sports Medicine specialty, it is our practice policy to offer patients with acute injury related conditions the soonest available appointment. This is usually on the same day or next available weekday. Please call reception and advise of your condition, so that we can triage accordingly.

As we do see acute problems, unavoidable delays do happen in the appointment schedule. We will endeavour to inform you of any delays as soon as possible.

If your problem is urgent, e.g. chest pain or shortness of breath, please tell the receptionist immediately.

If you are attending for an injury related condition, on most occasions our doctors will need to conduct a medical examination which will generally involve a series of movements. We therefore request that you wear non restrictive or revealing clothing.
Should an invasive procedure such as an injection be required, it’s important to attend the clinic with clean skin to reduce the risk of infection.

Our practice makes every effort to ensure healthcare remains accessible to all patients by charging well below recommended rates. We have intentionally kept the GAP for all consultations not involving additional tests or procedures to approximately $30 regardless of the length of time for the consultation. This does not apply to consultations with our Sports Physician Registrar.

Additional procedures required will add a further Gap Fee.

Payment is to be provided at the time of the consultation.
Payment options are cash, credit card or eftpos.
We are able to process your medicare rebate on the same day for most visits.

A $30 fee applies for non-attendance of scheduled appointments and appointments cancelled with less than 24 hours notice.

It is your responsibility to call the practice regarding your test results. However our staff will endeavour to contact you regarding any significant or concerning results. Results will only be given over the phone if this has been authorised by the doctor and only to the patient or parent of a child under 16 years.

This practice offers “recalls” & “reminders” for patients to promote continuity of care and to ensure best possible health outcomes.

We use SMS & EMAIL to contact you for proactive reminders and other health information. (This does NOT include results or other private information)

An Appointment with a Doctor is required for ALL script requests.

Please ensure that you contact us and make an appointment before your medication runs out.

Home visits are available for regular patients of this practice whose condition prevents them from attending the surgery and prior arrangement made with the individual doctor.

We are committed to providing teaching placements for health professional. This includes:

  • High school work experience

If you have any concerns with students being present during your consultation please let your doctor, nurse or receptionist know.
